## Frequently asked questions

**What EOB formats can Lemma convert?**

Scanned images, phone photos, and PDFs, whether they arrive by mail through your Lemma lockbox or you upload them yourself. Multi-page and multi-claim EOBs are split into their individual claims.

**What is an 835 file?**

The standard electronic remittance advice format. It carries the payer, the claim, the adjustment codes, and what was actually paid, and most billing platforms accept one directly.

**How accurate is the extraction?**

96% on first pass. Anything the model is not confident about is flagged for review with the original image beside it, rather than written into the file unchecked.

**Where do the original documents live?**

In Lemma, attached to the 835 and to the deposit they explain. They are encrypted at rest and available whenever a payer question, an appeal, or an audit comes up.

**Do I have to use the Lemma lockbox?**

No. The lockbox exists so paper never has to touch your office, but you can upload documents manually and they run through the same conversion and matching.

**Does this work for payments that did not come through Lemma?**

Conversion works on any EOB you send us. Automatic matching needs the payment to have landed in a Lemma account, since that is where the deposit data lives.

**Can I see what each provider collected?**

Yes. Converted remittance carries the rendering provider, so collections break down per provider. That is the collected figure, not the billed one, which is what productivity and compensation calculations need.

**What can I search on?**

Practically anything on either side of a payment: a trace number, an amount, a claim number, a patient or account ID, a payer name. Results come back as both the matching banking transactions and the matching claim data.

**Is EOB conversion HIPAA-compliant?**

Yes. Lemma is a HIPAA Business Associate and the platform agreement includes a Business Associate Agreement, so PHI on every document is handled under the Privacy and Security Rules.
