Lemma reads your explanation of benefits (EOB) documents for you. Every EOB becomes a remittance with its payer, claims, and amounts extracted, and Lemma matches it to the deposit that paid it. You stop keying EOBs into spreadsheets and cross-referencing bank statements.

Get EOBs into Lemma

Lockbox auto-parsing

Payers mail EOBs to your Lemma lockbox. Lemma scans and parses them with no action from you.

Upload documents

Upload EOBs you get by email, payer portal, or on paper. Drop in as many files as you like at once.
EOBs from a clearinghouse or EHR arrive as electronic remittances instead. See integrations.

What happens to each document

  1. Lemma splits it. One file often holds several EOBs, a check, and unrelated payer letters. Lemma separates it into sections and labels each one, such as an EOB, a check, or a denial letter.
  2. Lemma extracts every EOB. For each EOB section, Lemma pulls out:
    • Payer: the insurance company’s name, address, and tax ID
    • Payee: your practice as the payer lists it
    • Claims: patient, member ID, service dates, procedure codes, and the charged, allowed, paid, and patient responsibility amounts
    • Adjustments: the reason codes behind every amount the payer didn’t pay
    • Payment: the check or EFT number and the total paid
  3. The EOB appears as a remittance. Open Reconciliation in the sidebar to see it alongside your electronic remittances.
  4. Lemma matches it to a deposit. Lemma looks for the deposit that paid each remittance and links the two automatically.

Download an 835

Lemma rebuilds every parsed EOB as an X12 835 file. Open the remittance and select Download 835 above the EOB preview. Use it to load a paper EOB into an EHR or practice management system that only accepts 835 files.

Reconcile and post remittances

Review matches, mark remittances as posted, and handle payments that don’t match.
