How reconciliation works
When you view a parsed EOB in the Payer Remittances table, Lemma searches your recent transactions for deposits that match the EOB’s total payment amount. Matching transactions are displayed alongside the EOB details so you can confirm the connection in one place. This replaces the manual process of:- Reading the total payment amount from an EOB
- Opening your bank statement
- Searching for a deposit with that amount
- Cross-referencing dates and payer names to confirm it is the right one
Reconciling a payment

Go to the EOB section
Navigate to the EOB page and select the Insurance payments tab.

Select a payment
Choose a parsed EOB from the list. The detail panel shows the full breakdown —
payer, claims, and total amount.

Review matching transactions
Lemma displays transactions with amounts that match the EOB’s payment total.
Review the transaction details (date, counterparty, amount) to confirm it is
the correct deposit.

Confirm the match
Once you identify the matching transaction, confirm the reconciliation. The
EOB and transaction are linked together for your records.
Downloading an 835
Once Lemma has parsed a lockbox or uploaded EOB, you can download the equivalent 835 EDI file from the remittance detail view. Use this when your EHR or clearinghouse accepts 835 files but the payer sent you a paper EOB instead.- Open the EOB page and select the Insurance payments tab.
- Choose a parsed remittance.
- Click Download 835.
Provider-level adjustments
For remittances sourced from an ERA or 835 file, the check total may not equal the sum of claim payments. Provider-level balance (PLB) adjustments account for the difference. These include:- Recoupments — the payer clawing back a prior overpayment
- Forwarding balance — a balance carried into or out of this check
- Interest — interest the payer owes on late payments
- Capitation — periodic capitated payments outside any single claim
- Withholding — funds withheld against a future obligation
Lockbox payments
When an insurance company mails both a check and an EOB to your lockbox, the process is even simpler. The check is deposited automatically and the EOB is parsed automatically. Lemma links the check deposit to the parsed EOB, so many lockbox payments arrive pre-reconciled.Tips for reconciliation
- Check amounts first — the total payment amount is the primary matching criterion
- Verify the payer — confirm the transaction counterparty matches the insurance company on the EOB
- Allow for timing differences — a check deposit may settle a few days after the EOB is dated, so the transaction date and EOB payment date may not match exactly
- Bulk payments — some payers send a single check covering multiple EOBs. In this case, the deposit amount will be the sum of several EOB payment totals
- Check totals that don’t match the claims — for ERA-sourced remittances, look at the Provider-Level Adjustments section on the detail page. Recoupments, forwarding balances, and interest are applied outside any single claim
